The Waiver Program of Upstate Cerebral Palsy includes a variety of services funded by Medicaid waivers to provide support and service to people with disabilities and their families. Programs which are part of the Waiver Services include:
Residential Habilitation: working with people to enhance skills necessary to perform daily activities that occur in an individual’s home such as personal grooming, household chores, cooking, communication skills and social skills.
Day Habilitation: working to enhance the skills necessary for daily activities that occur outside of the home such as shopping, travel, training money management and socialization skills.
Respite: relief provided to the caregiver of someone with a disability. This primary care and support is available on an hourly basis and is provided at the home of the caregiver.
Environmental Modification: coordination of necessary changes in a family’s home to enable greater independence for an individual with a disability through the addition of supports such as ramps, lifts, widened doorways or bathroom modifications.
Assistive Technologies: help in obtaining adaptive or communication devices, which are necessary to live at home with greater independence and safety.
Eligibility Requirements
Children and adults who have a developmental disability and require assistance and support in areas of Independent Living, Activities of Daily Living, Communication, and Socialization.
