Medicaid Service Coordination (MSC) works with an individual to help them explore what they want and need in life and then assist them in getting it. Working in partnership with the person or their family, MSC will work to explore the individual’s strengths, capacities and goals for the future. Once a plan is in place, the MSC will help to secure the supports and services necessary to meet these goals. These services are provided by Upstate Cerebral Palsy in Oneida, Lewis, Herkimer and Madison Counties.

Persons with a developmental disability.

Service Coordination is funded through Medicaid.
Where and when are the services provided?
Coordination Services are provided in the persons home and in the community at a minimum of one time per month. Services are provided on an agreed upon day and time. The people have the ability to choose their service coordinator and have the right to change service coordinators at any time they feel it is necessary.
