Assistant Residence Manager
Oversee daily shift operations, ensure accomplishment of goals, coordinate staff orientation and training, complete all records and reports, accomplish contacts with outside vendors, perform the duties of the Residence Counselor, monitor the operation and condition of the building and supplies, and participate in Agency activities.
Behavioral Specialist
Provide assessment of intellectual and emotional capabilities, evaluate behavioral issues, develop and implement behavior plans, complete required documentation, furnish data for psychiatric consults, develop and implement in-service training, educate treatment team members and participate in Agency activities.
Behavior Specialist Assistant
Evaluate behavioral issues, assist with the development and implementation of behavior plans, complete required documentation, furnish data for psychiatric consults, develop and implement in-service training, assist in education treatment team members and participate in Agency activities.
Case Manager
Conduct and schedule assessments and evaluate needs, develop an effective communication network, provide referrals, provide advocacy and counseling, educate individuals and team members, ensure compliance with state, federal and Agency regulations and operating standards, complete all records and reports, participate in Agency activities and provide transportation as required.
Certified Occupational Therapy Assistant
Assist with program participant evaluations and assessments, educate individuals and team members, complete required documentation, communicate with other health care professionals, assist with departmental maintenance and supplies, participate in Agency activities, assist in orientation to the department and assist with equipment adaptation.
Cleaner
Provide cleaning of assigned areas, maintain supplies, remove furniture and equipment as needed, adjust heat or air conditioning, perform building checks (daily, weekly and monthly) and participate in Agency activities.
Compliance Review Specialist
Responsible for conducting internal compliance reviews as part of the organization's overall corporate compliance program. Specialist will conduct all internal compliance reviews, develop recommendations based up review findings, prepare review protocols, risk assessments, reports. Act as liaison will all government agencies conducting reviews of Upstate Cerebral Palsy programs.
Cook
Responsible to prepare meals, set the dining area, clean the dining and kitchen areas, clean appliances, promote program participant involvement, purchase groceries and participate in Agency activities.
Community Habilitation Aide
Implement the individual habilitation plan, attend to individual's personal care needs, respect the individual's privacy and home environment, participate in Agency activities, provide transportation as requied and maintain documentation.
Day Care Aide
Responsible to assist the teacher in coordinating classroom activities, participate in classroom maintenance and personal care, assist in training new aides, assume duties of lunch/float aide and to assist the therapist in carrying out goals.
Day Care Teacher
Maintain records, plan lessons and activities, plan and prepare snacks and serve lunch, perform personal care, compile reports and related paperwork and participate in Agency activities.
Developmental Disabilities Specialist
Assist in special projects and determine residential and day treatment training needs related to special projects; and make necessary arrangements, recommend changes and improvements to the program, ensure compliance with regulations, develop new materials and resources, maintain required documentation, implement quality assurance activities and participate in Agency activities.
Director of Residential Services
Serves as a role model, maintaining and supporting positive relationships with staff in an environment of mutual respect and trust. Director will provide supervision & oversight to several residences serving indidividuals with special needs.
Fiscal Manager
Responsible for administering financial and statistical data for assigned programs. This includes, but is not limited to generating billings, required reports, budgets, quarterlies, financial reports and data, performing all daily activities relating to their assigned programs and other assigned tasks as required.
Housekeeper
Accomplish all housekeeping tasks throughout the facility, wash, dry and fold laundry, report house damage and participate in Agency activities.
Human Services Aide
Assist in coordinating classroom activities, maintain paperwork, assist with community outings, provide assistance with personal care activities, participate in Agency activities and maintain kitchen and supplies.
Human Services Practitioner
Responsible to oversee group and individual activities, maintain documentation, coordinate the classroom team, plan recreation activities, provide assistance with personal care activities, participate in Agency activities, provide staff orientation and maintain kitchen and supplies.
Licensed Practical Nurse
Provide daily nursing care, order medications, provide updates to the RN and Nurse Practitioners, train and orient AMAP’s, schedule appointments, participate in Agency activities and complete required documentation.
Job Coach
Coach individuals, lead weekly VIP program, assist individuals in developing social skills, respond to requests for individuals' personal needs, maintain treatments and programs, promote integration, maintain documentation and participate in Agency activities.
Maintenance Mechanic
Responsible to accomplish assigned duties in masonry, carpentry, electrical and painting, perform efficiency tests, general ground and building maintenance, perform preventative maintenance and participate in Agency activities.
Maintenance Specialist
Supervise assigned departmental activities. Assess cost estimates for building and maintenance and capital improvement projects, update preventative maintenance records monthly, quarterly and annually, as required by state regulations. The Specialist will also ensure adequate supplies for maintenance and housekeeping staff. Responsible for assisting with the annual evaluations of the job performances of maintenance workers and cleaners.
Maintenance Worker I
Assist with semi-skilled work in masonry, carpentry, electrical and painting, general ground and building maintenance, perform preventative maintenance and participate in Agency activities.
Maintenance Worker II
Responsible to accomplish assigned duties in masonry, carpentry, electrical and painting, perform efficiency tests, general ground and building maintenance, perform preventative maintenance and participate in Agency activities.
Medical Office Assistant
Type and maintain correspondence, medical transcription, maintain files including medical records, generate and maintain reports, process telephone calls and intakes, scheduling of medical appointments, handle mail, research and requisition supplies and promote positive communications, and coordinate initial billing intake and authorization.
Medicaid Service Coordinator
Assist persons with developmental disabilities and mental retardation in gaining access to necessary services and suipports appropriate to the needs of the individual. Uses a person-centered planning process in developing, implementing and maintaining an Individualized Service Plan. Promotes and advocates for the concepts of choice, individualized services and supports and consumer satisfaction.
Mental Health Advisor
Promote the individuals recovery, provide support to program participants, promote program participant input, oversee safety, maintain the facility, equipment and supplies, participate in Agency activities, maintain documentation and transport and accompany program participants in a residential, treatment and/or a community support setting.
Mental Health Peer Support Specialist
Promote the individuals' recovery, provide support to the program participants, promote program participant input, oversee safety, maintain the facility, equipment & supplies, participate in Agency activities and maintain documentation and transport and accompany program participants in the MICA network to residential, treatment and/or community support settings.
Mental Health Professional I
Promote the individual's recovery, promote consumer involvement, provide counseling, provide crisis intervention, ensure compliance with governmental and Agency regulations and operating standards, complete all records and reports, act as a liaison, provide support and consultation, promote program participant input, transport and accompany program participants and participate in Agency activities.
Mental Health Professional II
Provide clinical supervision, promote the individuals' recovery, promote consumer involvement, provide counseling, provide crisis intervention when appropriate, ensure compliance with governmental and Agency regulations and provide support and consultation, promote program participant input, transport and accompany program participants and participate in Agency activities.
Occupational Therapist - Registered
Responsible to evaluate and assess program participants, educate individuals and team members, complete required documentation, communicate with other health care professionals on the program participants' IPP/IEP.
Physical Therapist - Registered
Evaluate and assess program participants, educate individuals and team members, complete required documentation, communicate with other health care professionals on the program participants' IPP/IEP, assist with departmental maintenance and supplies, participate in Agency activities, assist in orientation to the department and assist with equipment adaption.
Registered Physical Therapy Assistant
Responsible to assist with program participant evaluations and assessments, educate individuals and team members, complete required documentation, communicate with other health care professionals regarding the IPP/iEP, assist with departmental maintenance and supplies, partcipate in Agency activities, assist in orientation to the department and assist with equipment adaptation.
Program Services Specialist
Develop each individual’s IPP, ensure service implementation, schedule and conduct interdisciplinary team meetings, complete assessments, evaluations and records, act as a liaison, monitor services, participate in Agency activities and provide monthly QA.
Recreation Counselor
Develop appropriate recreation activities, participate a treatment team meetings, generate a monthly recreation calendar, oversee community outings, utilize appropriate equipment, maintain all documentation, attend to individual's personal, self-care needs, advise nurses regarding medical and dietary concerns and participate in Agency activities.
Recreation Specialist - Certified
Develop appropriate recreation activities, participate at treatment ream meetings, generate a monthly recreation calendar, oversee community outings, utilize appropriate equipment, maintain all documentation, attend to individuals' personal, self-care needs, advise nurses regarding medical and dietary concerns and participate in Agency activities.
Registered Nurse
Assess and monitor medical concerns, review and provide nursing care to program participants, complete required documentation, provide in-services to staff, collaborate with physicians and nurse practitioners, certify AMAP’s, review and act on incident reports, provide on-call coverage and participate in Agency activities.
Residence Coordinator I and II
Supervise daily program operations, oversee the interdisciplinary team process, coordinate services, maintain and effective communications network, ensure active treatment, monitor facility conditions and supplies, ensure compliance with state, federal and Agency regulations and operating standards, complete all records and reports and participate in Agency Activities.
Residence Counselor
Implement the IPP, attend to individuals’ personal care needs, oversee the individuals’ safety, keep nurses advised, participate in Agency activities, transport and accompany individuals’ on activities and assist in daily housekeeping and other facility-related duties and maintain documentation.
School Aide
Assist the teacher in coordinating classroom activities, participate in classroom maintenance and personal care, assist in training new aides, assume duties of lunch/float aide and assist the therapist in carrying out goals.
Secretary
Responsible to type and maintain correspondence, attend committee meetings and take minutes, maintain files, generate and maintain reports, process telephone calls, assist in scheduling, handle mail, research and requisition supplies, participate in agency activities, and promote positive communications.
Social Worker
Process referrals, perform screening, conduct volunteer orientations, recruit new volunteers, advise and participate in fund raising, complete all required paperwork, and participated in Agency activities.
Social Worker – MSW/CSW
Promote the individuals’ recovery, promote consumer involvement, provide counseling, provide crisis intervention, ensure compliance with governmental and Agency regulations and operating standards, complete all records and reports, act as a liaison, provide support and consultation, promote consumer input, transport and accompany program participants and participate in Agency activities.
Special Education Teacher
Sees that each child’s IEP is carried out, takes part in interdisciplinary team meetings, oversees classroom aids and assistants, compiles reports and statistics, maintains supplies and participates in Agency activities.
Speech-Language Pathologist
Evaluate and assess program participants, educate individuals and team members, complete requir4ed documentation, communicate with other health care professionals on the program participants’ IPP/IEP, assist with departmental maintenance and supplies, participate in Agency activities, assist in orientation to the department and assist with equipment adaptation.
Speech Therapist
Evaluate and assess program participants, educate individuals and team members, complete required documentation, communicate with other health care professionals on the program participants’ IFF/IEP, assist with departmental maintenance and supplies, participate in Agency activities, assist in orientation to the department and assist with equipment adaptation.
Teacher Aide
